This website provides general information about Sunniva Counseling and services offered by Angela Sloan, LCSW and LMSW, for adults and couples, including Mesa, Arizona couples intensives and virtual therapy in Arizona, Colorado, Texas, Louisiana, and Iowa. Use it lawfully. Do not disrupt its operation, submit another person’s information without permission, or send abusive or misleading messages.
Information is not individualized clinical care
Website content is educational and informational. It is not a diagnosis, an individualized treatment plan, or a promise of outcomes. Services, availability, suitability, and scheduling must be confirmed with the practice. A consultation request, website agreement, or booking request does not itself create a therapist–client relationship or guarantee an appointment. Any professional relationship requires the practice’s separate intake and service agreements.
Enquiries and permission to respond
When you submit an enquiry, you authorize Sunniva Counseling to respond using the contact details you provide about that request, consultation, and related scheduling. This is operational contact permission, not consent to unrelated marketing or promotional messages. Optional campaign storage is a separate choice and is not required for enquiries or booking. Provide accurate details and avoid sensitive clinical histories in this initial form.
You must complete required fields. A successful website response means the CRM endpoint accepted the request; it does not guarantee clinical acceptance or that every field was retained. If an error appears, your entries remain so you can retry. If you are unsure whether a request arrived, contact the practice before resending to avoid duplicate enquiries.
Calendar, appointments, and payment questions
The external LeadConnector (HighLevel) calendar loads only after a separate agreement in the booking area. Review its displayed information before choosing an appointment. The practice confirms availability and suitability. Intensive dates, deposits, fees, cancellation, and rescheduling terms must be discussed with the practice; this website does not process deposits or establish a cancellation schedule. Separate service terms may apply when confirmed with you.
Third-party tools and website agreements
Enquiries go directly from your browser to a LeadConnector (HighLevel) CRM webhook. Booking uses its external iframe and resizing script. Hosting, fonts, and external links are described in our Privacy Policy. These services may have their own conditions and privacy practices. Sunniva Counseling cannot guarantee their continuous availability or control all behavior inside the external calendar.
Clicking an agreement button records acceptance and policy acknowledgement in the page’s memory. Submitted forms include the terms version, a browser-generated agreement timestamp, and agreement wording. CRM retention of these fields has not been verified. The website does not claim the external calendar retains the website agreement.
Not for emergencies
Forms, email, and the booking tool are not monitored emergency channels. If you or someone else is in immediate danger, call 911. In the United States, call or text 988 for suicide and crisis support. Do not wait for a website response when urgent help is needed.
Appropriate limits and updates
We aim to keep website information accurate but do not guarantee that all information is current, that the site will always be available, or that using it will produce a particular result. These website terms do not waive rights or responsibilities that cannot lawfully be waived. Clinical consent, privacy obligations, and any professional service agreement are separate. Updated website terms will carry a new version and effective date; review the current text before making a new request.
